The student’s application file will be complete and entrance testing will be scheduled on a individual basis after we have received the following forms (and any fees requested). Parents will find the “Directions to Apply” helpful in moving through the application process. Below we have provided all the necessary forms in both PDF and WORD formats. If you use the PDF format, you must download the form to print out and complete by hand. If you use the WORD format, you may complete the form on your computer screen, then print out and sign with a “live” signature and mail or fax to CCA. The required application fee must accompany the Application for Admission.
